A Complete Chart Review reads every page of your service and civilian medical record and identifies every condition you can claim — named, coded, and matched to the Statement of Principles that governs it. Fixed fee $600 + GST.
Most veterans claim what they remember. A chart review finds what is documented. Conditions sit recorded across hundreds of pages that nobody has read end to end — an entry on a deployment, a physio referral, a line in a separation medical. Those entries are the evidence a claim is decided on, and they are worth nothing if nobody finds them.
It is ordinary for a review to return several times the number of conditions a veteran came in expecting.
Service medical records from Defence, and your civilian record from your GP. If you do not have them, we will tell you exactly what to request and from whom — see how to access your ADF medical records and your DVA records.
Every page is read and indexed. Nothing is skimmed and nothing is sampled.
You receive a written document listing every claimable condition, with the ICD-10 code, the date of clinical onset, the pages in your record that support it, and the Statement of Principles that applies.
A chart review is not the claim. It is what makes the claim possible: the Diagnostic Assessments, the permanent impairment assessment, and any appeal all work from it. Done once, properly, it does not need doing again.
You can see the standard before you pay for it — the Claims Library holds de-identified example reports across hundreds of conditions.
