The veteran, Airfield Defence Guard, enlisted 29 July 1993, currently serving.
Occupational History
As an Airfield Defence Guard (ADG) in the Royal Australian Air Force, the veteran was exposed to significant physical stressors including manual handling of heavy equipment, weapons, ammunition, and field gear. His duties involved lifting, carrying, and pack marching, often over uneven terrain and for extended periods. His role required physical training, fieldcraft, and participation in exercises and operational activities which placed repetitive stress on the musculoskeletal system, particularly the lower limbs. Health Surveillance Questionnaires consistently document exposure to physical demands and note his involvement in weapons range activities and proximity to flight lines. The veteran's occupation required frequent kneeling, squatting, and repetitive movements that placed considerable stress on the knee joints.
History
The veteran the veteran an Airfield Defence Guard in the RAAF, was diagnosed with quadriceps fat pad impingement (Hoffa's syndrome) of the right knee, identified on MRI examination on 04 December 2018. This condition likely developed as a secondary issue following his complex history of right knee injuries, including the initial ACL rupture in 2003 and subsequent surgical interventions.
Timeline
- 06 Jul 1999. The veteran sustained a significant injury to his right knee while playing touch football as part of a service sporting activity. He reported hearing a loud crunch and pop with immediate pain and swelling. An MRI performed that day revealed a ruptured anterior cruciate ligament and microtrabecular fracturing of the posterolateral tibial condyle. This traumatic event was the initial injury to the right knee that would later develop impingement syndrome.
- 24 Jul 1999. The veteran underwent right knee ACL reconstruction performed by the treating doctor. During the procedure, a small tear on the inner one third of the posterior horn of the lateral meniscus was debrided. This surgery represented the first intervention to the knee joint structures.
- 19 Feb 2000. Due to persistent symptoms, the veteran underwent arthroscopic examination of his right knee by the treating doctor the procedure, some scar tissue in the anterior part of the joint in the intercondylar notch area was resected. This indicates early abnormal tissue changes occurring in the knee.
- 17 Feb 2007. The veteran underwent a right revision anterior cruciate ligament reconstruction by the treating doctor, using a patella tendon autograft. This second major surgical intervention to the knee further altered the biomechanics of the joint.
- 30 Apr 2009. An MRI of the Right Knee was performed following re-injury. The report stated: "The ACL graft is impinged..." with high signal within the graft. This demonstrated ongoing impingement issues developing within the knee.
- 04 Dec 2018. An MRI of the Right Knee documented multiple findings including: "Minor quadriceps fat pad impingement" (Hoffa's Syndrome). This is the first definitive diagnostic imaging evidence confirming the fat pad impingement condition.
Symptoms
At the time of initial injury in September 2001, the veteran experienced acute pain, swelling, and loss of function in the right knee. Following his ACL reconstruction and subsequent surgeries, he experienced persistent symptoms including stiffness, impaired extension, crepitus, and pain.
Current symptoms associated with the quadriceps fat pad impingement include anterior knee pain that worsens with extension of the knee, tenderness in the anterior aspect of the knee, and a sense of fullness or swelling in the anterior knee. The patient experiences difficulty with squatting, kneeling, and climbing stairs. The condition contributes to discomfort with prolonged standing and walking. - 04 Dec 2018: MRI of the Right Knee (as part of Both Legs and Knees study): "Previous ACL reconstruction...The ACL is intact. No recurrent tear. No knee joint effusion...minor chondromalacia [weightbearing osteochondral surfaces medial and lateral compartments]. Marginal osteophytes within medial compartment suggestive of early OA changes. Patellofemoral joint demonstrates grade 2 chondromalacia with chondral thinning, fissuring and high signal intensity especially at the apex of the patella...Minor quadriceps fat pad impingement."
