The veteran, Airfield Defence Guard, enlisted 29 July 1993, currently serving.
Occupational History
As an Airfield Defence Guard (ADG) in the Royal Australian Air Force, the veteran was exposed to a range of occupational hazards inherent to the role. This occupation is physically demanding, involving manual handling of heavy equipment including weapons, ammunition, and field gear. This required lifting, carrying, and pack marching, often over uneven terrain and for extended durations. ADG duties involve extensive use of hands for weapons handling, equipment operation, and various physical tasks that place repetitive stress on finger joints. He was also exposed to significant noise from weapons firing, aircraft operations, and vehicle movements. Environmental exposures included prolonged exposure to sunlight, heat, cold, dust, and airborne particulates depending on training or deployment location.
History
The veteran an RAAF Airfield Defence Guard, sustained a crush injury to his right index finger in late January/early December 2019 while working on the fire section. This traumatic event initiated a sequence leading to the diagnosis of osteoarthritis in the distal interphalangeal joint (DIPJ) of his right index finger.
Timeline
- Late Jan/Early Dec 2019: The veteran sustained a crush injury to the base of his right trigger finger (index finger) while on the fire section at work, when a tank belonging to another person crushed his finger. He reported soreness at the time but did not seek immediate medical attention as he thought it would resolve.
- 22 Nov 2017: Presented to sick parade reporting that his right index finger was weak and he was finding it difficult to open things like jars, approximately 7 weeks after the crush injury. Examination revealed the finger looked normal, was not red, hot, or swollen, and had a full range of motion. He was referred for X-ray and ultrasound.
- 26 Nov 2017: Ultrasound of the right index finger showed no acute injury, with unremarkable metacarpophalangeal and interphalangeal joints, intact collateral ligaments, and unremarkable extensor and flexor tendons and pulleys. X-ray of the right hand noted mild degenerative change of the interphalangeal joint.
- 04 Dec 2018: MRI of the Right Index Finger revealed no acute bone marrow edema or fracture, but showed reduction in the joint space within the DIPJ of the right index finger suggesting early osteoarthritic changes. The collateral ligaments, flexor and extensor tendons, and pulley mechanism appeared normal. The conclusion stated "Very minor degeneration at the DIPJ."
Symptoms
At the time of the initial injury in early 2022, the veteran experienced soreness in his right index finger but did not consider it severe enough to seek immediate medical attention.
After approximately 7 weeks, he developed progressive symptoms including weakness in the right index finger and difficulty with gripping activities, particularly opening objects like jars. This functional impairment prompted him to seek medical evaluation.
Currently, the veteran experiences periodic pain in the distal joint of his right index finger, particularly with use, mild stiffness that worsens with cold weather, reduced grip strength, and difficulty with fine motor tasks requiring precision grip. He has noticed some enlargement of the joint compared to his other fingers, and experiences occasional catching or locking sensations with certain movements. The symptoms impact his occupational duties as an Airfield Defence Guard, where hand dexterity and strength are essential for weapons handling and equipment operation.
Imaging
- 26 Nov 2017: X-ray of the right hand: "Mild degenerative change of the interphalangeal joint of the thumb. No other significant arthropathy identified. No fracture or dislocation." (It should be noted that while the report mentions the thumb, the clinical indication was for the right index finger.)
- 26 Nov 2017: Ultrasound of the right index finger: "The metacarpophalangeal and interphalangeal joints of the index finger are unremarkable. No effusion. Collateral ligaments intact. The extensor and flexor tendons and pulleys are intact and unremarkable." Conclusion: "No acute injury identified."
- 04 Dec 2018: MRI of the Right Index Finger: "No acute bone marrow oedema or fracture. There is reduction in the joint space within the DIPJ of the right index finger which suggests early OA changes." Conclusion: "Very minor degeneration at the DIPJ."

This crush injury meets the SOP definition of trauma as it involved application of significant physical force to the affected joint, caused damage, resulted in symptoms within 24 hours, and these symptoms persisted.

These activities would have required both repetitive movements and sustained gripping actions affecting the finger joints, including the right index finger DIPJ. The occupational demands easily exceed the threshold of at least 100 hours per month for a cumulative period exceeding ten years.
inability to obtain appropriate clinical management for osteoarthritis - PARTIALLY MET
- There was a delay between the initial injury (January/December 2019) and the comprehensive assessment with MRI (February 2021). The Full Federal Court in Brew v Repatriation Commission (14 May 1993) established that "inability" encompasses both objective and subjective barriers to obtaining treatment. Sequelae

Inability to Attain Appropriate Medical Management

While he did receive initial evaluation following his delayed presentation in January 2020, the comprehensive assessment with MRI that definitively identified the degenerative changes did not occur until February 2021, approximately 14 months after the initial injury.
The Full Federal Court in Brew v Repatriation Commission (14 May 1993) established that "inability" to obtain appropriate clinical management should be interpreted broadly, encompassing both objective limitations (like availability of services) and subjective barriers (such as the veteran's perception of the injury severity or competing priorities).
In the veteran case, several factors may have contributed to this delay:
- His initial self-assessment that the injury would resolve without medical attention
- The focus of initial evaluation on ruling out acute injury rather than assessing for degenerative changes
- The subtle progression of early osteoarthritic changes that may not have warranted immediate advanced imaging
This delay in comprehensive assessment and targeted management would likely have contributed to the progression of the degenerative process, resulting in a permanent worsening of the condition beyond what might have occurred with earlier intervention.
